SAMHS Fund contributors, a main member with spouse /life partner, are covered for R25,000,00 each.
For Out of the Force members and Life members, the annual premium is payable on the first working day of January or the 15th day as requested by the member. Pro-rata payments will be due in case the policy is in arrears. It is recommended that contributors pay by debit order initiated by SAMHS Fund.
Please take note: Out of the Force and Life Members must apply to continue with the Funeral Policy as this is not an automatic transaction and not free of charge.
The full amount of R25 000 will be paid to a beneficiary in the event of the insured main member/spouse or life partner passing on. Documentation in this regard will be submitted to the Insurance Company, Siyavika, via the SAMHS Fund Office upon receipt thereof from the Claimant. Please submit the following documentation for a claim:
- The original Claim Form,
- A certified copy of the death certificate,
- A certified copy of the deceased’s identity document,
- A certified copy of the claimant’s identity document,
- A certified copy of the marriage certificate,
- A copy of the Bl1663 – Notice of Death,
- A police report in the case of death due to unnatural causes,
- Proof of banking details of the claimant, and
- If the claimant is a different person/entity from the beneficiary, please attach a written consent form from the Master of the High Court.
- Also, an affidavit from the SAPS confirming the relationship with the deceased.

- The option of adding your relatives is not for free. Additional premiums are charged. Siyavika is responsible for deducting the premium payments from your bank account.
